Trichomoniasis is a sexually transmitted infection (STI) caused by a parasite called Trichomonas vaginalis. It mainly affects the vagina or urethra and is usually passed on through vaginal sex, or by sharing sex toys without washing them or covering them with a new condom between uses.
Many people have no symptoms. When symptoms are present, they can be similar to other STIs or vaginal infections, so a test is needed to confirm the cause rather than relying on symptoms alone.
Trichomoniasis can be treated, but reinfection is possible. Current sexual partners also need assessment and treatment so that the infection is not passed back and forth.
This service may be appropriate if you are aged 18 or over and you have a confirmed positive trichomoniasis result, or a current sexual partner has received a confirmed diagnosis and you have been advised to seek treatment. The prescriber may ask for further information before deciding whether online treatment is appropriate.
Arrange testing through a sexual health clinic or GP if you have symptoms but have not had a confirmed test. Clinic assessment is also more appropriate if you are pregnant, may be pregnant or are breastfeeding, have pelvic or lower abdominal pain, have testicular pain or swelling, have a fever or feel significantly unwell, have symptoms that could have another cause, have already used treatment but your symptoms have persisted or returned, or need testing for other STIs.
Seek urgent medical advice if pain is severe, symptoms are rapidly worsening or you feel acutely unwell.
Trichomoniasis is caused by a parasite called Trichomonas vaginalis. In adults, it is passed on mainly through sexual contact involving the penis and vagina.
It can also be passed on by sharing sex toys if they are not washed or covered with a new condom between users. It is not usually passed on through kissing, hugging, toilet seats, cutlery or other ordinary social contact.
Because trichomoniasis frequently causes no symptoms, someone can pass it on without knowing they have it. Using condoms and not sharing unwashed sex toys can lower the risk, but testing is the only way to confirm whether an infection is present.
Many people with trichomoniasis have no symptoms. When symptoms occur, they may begin several days after exposure or much later.
In women (or anyone with a vagina), symptoms may include more vaginal discharge than usual, thin, frothy or yellow-green discharge that sometimes has an unpleasant smell, itching, soreness, swelling or redness around the vagina, pain when passing urine, and discomfort during sex or lower abdominal discomfort.
In men (or anyone with a penis), symptoms may include discharge from the penis, pain or burning when passing urine, needing to pass urine more often, soreness or irritation around the penis, and discomfort during ejaculation.
These symptoms are not specific to trichomoniasis. If you have symptoms without a confirmed positive test, arrange assessment and testing through a sexual health clinic or GP rather than self-selecting treatment.
Trichomoniasis is treated with an oral antimicrobial medicine. A seven-day course of metronidazole is the recommended UK regimen and is the option offered through this service, subject to an individual assessment.
Complete the full course exactly as prescribed, even if symptoms improve sooner. Do not drink alcohol while taking metronidazole or for 48 hours after your final dose.
Current sexual partners should be assessed and treated at the same time. Do not have sex until at least seven days after you and your partner or partners have completed treatment and any advised follow-up.
Anyone diagnosed with trichomoniasis should also be offered testing for other STIs. If symptoms do not improve, return after treatment, or you think you may have been exposed again, contact a sexual health clinic or GP for reassessment rather than taking another course without review.
Do I need a positive test before requesting treatment?
This is a diagnosis-led service. You should normally have a confirmed positive trichomoniasis result, or be a current sexual partner of someone with a confirmed diagnosis who has been advised to seek treatment. If you have symptoms but have not been tested, arrange testing through a sexual health clinic or your GP.
Can trichomoniasis cause no symptoms?
Yes. Many people do not notice any symptoms but can still pass the infection on. Symptoms alone also cannot confirm trichomoniasis, because several other infections cause similar problems.
Can both women and men get trichomoniasis?
Yes. Trichomoniasis can affect people with a vagina or a penis. It commonly affects the vagina or urethra, although symptoms and the most appropriate testing method can differ.
Does my partner need treatment?
Current sexual partners should be assessed and treated at the same time, even if they have no symptoms. They will need to order separately. A sexual health clinic can also help with confidential partner notification, including options that do not identify you.
When can I have sex again?
Do not have sex until at least seven days after you and your current partner or partners have completed treatment and any advised follow-up. This reduces the risk of passing the infection back and forth.
Should I be tested for other STIs?
Yes. Other STIs can occur at the same time, so a full sexual health screen is recommended following a trichomoniasis diagnosis. A sexual health clinic can advise which tests are appropriate and when to take them.
Can I request treatment if I am pregnant or breastfeeding?
Not through this service. Pregnancy and breastfeeding change both the assessment and the follow-up required. Tell us if you are pregnant, may be pregnant, are trying to conceive or are breastfeeding, and we will direct you to your GP or a sexual health clinic for treatment and follow-up.
Can trichomoniasis come back after treatment?
Yes. Reinfection can occur if a sexual partner has not been treated, or if you are exposed again. Persistent symptoms can also have another cause. Seek reassessment rather than repeating treatment without advice.
What should I do if my symptoms do not improve?
Contact a sexual health clinic or your GP if symptoms persist after you complete treatment, return later, or become worse. They may need to check adherence, possible reinfection, another infection, or a less common treatment failure.
Can trichomoniasis clear without treatment?
It should not be left untreated. The infection can persist, be passed to other people, and may be associated with further sexual and reproductive health problems. Arrange testing and appropriate treatment if you think you may have been exposed.
