Which treatment is right for me?
Foundayo® may be suitable for people who want a daily tablet with no food, drink or timing restrictions, and who would prefer not to use injections.
Wegovy® Pill is also a once-daily tablet, but it must be taken immediately after waking with a small amount of water, followed by a 30 minute wait before eating, drinking or taking other medicines.
Wegovy® Injection contains the same active ingredient as Wegovy® Pill but is administered once weekly, making it a convenient option for many patients.
Mounjaro® works slightly differently and has demonstrated the greatest average weight loss in clinical studies, making it a popular choice for patients seeking the strongest weight loss results currently available.
Our clinicians will assess your medical history, treatment preferences, and weight loss goals to help determine the most appropriate option for you.
Foundayo® is the brand name for orforglipron, a once-daily prescription tablet developed by Eli Lilly for weight management in adults. It is a GLP-1 receptor agonist, taken alongside a reduced-calorie diet and increased physical activity.
Unlike other oral treatments in this class, Foundayo® can be taken at any time of day, with or without food, and with no restriction on when you eat, drink or take your other medicines.
How Foundayo® Works
Foundayo® acts on receptors for GLP-1, a hormone your body produces naturally after eating. This can help reduce appetite, increase feelings of fullness, and support a lower calorie intake. It works alongside changes to your diet and activity rather than replacing them.
How Effective Is Foundayo®?
In the ATTAIN-1 trial, adults with obesity taking the highest strength of orforglipron for 72 weeks achieved average weight loss of approximately 12.4%, compared with around 1% on placebo. Around 60% of participants lost at least 10% of their body weight, around 40% lost at least 15%, and around 20% lost at least 20%.
Results vary from person to person and depend on the dose you reach, how well you tolerate treatment, and the changes you make to your diet and activity.
Who Can Use Foundayo®?
Foundayo® is licensed for adults who:
- have obesity (BMI of 30 kg/m² or above), or
- are overweight (BMI of 27 kg/m² or above) and have at least one weight-related health condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, or pre-diabetes
Our clinicians will assess:
- your BMI
- weight-related health conditions
- medical history
- current medications
- previous weight loss treatments
We may also ask for photographs, recent measurements, identity confirmation or details from your GP record before we make a decision.
Foundayo® and Type 2 Diabetes
Foundayo® also holds a separate licence for type 2 diabetes. The treatment described here is supplied through our weight management service. If you have diabetes, do not start, stop or change your diabetes treatment without speaking to the clinician responsible for your diabetes care.
Foundayo® should be taken exactly as prescribed by your clinician and in line with your dispensing label.
How to Take Foundayo®
Foundayo® is designed to fit around your day rather than the other way round:
- Take one tablet once daily.
- Take it at any time of day, with or without food.
- Swallow the tablet whole. Do not split, crush or chew it.
- Do not take more than one dose in a day.
- Taking it at roughly the same time each day makes it easier to remember.
Your Dose Progression
You will start on the lowest strength and move up gradually. Increasing slowly gives your body time to adjust and helps reduce digestive side effects.
| Stage | Usual daily dose | Minimum time before a possible increase |
| Starting dose | 0.8mg | 30 days |
| First increase | 2.5mg | At least 30 days |
| Further increase | 5.5mg | At least 30 days |
| Later strengths, if needed and tolerated | 9mg, then 14.5mg, then 17.2mg | At least 30 days at each strength |
The maximum dose is 17.2mg once daily. Your clinician decides whether and when to increase your dose. You will not necessarily need the highest strength, and an increase may be delayed if you are experiencing side effects.
If You Miss a Dose
Take the missed dose when you remember, as long as you have not already taken a dose that day. Do not take two tablets in one day and do not double your next dose. Continue as normal the following day.
If You Are Having Surgery or a Procedure
Tell the healthcare professional responsible for your procedure that you take Foundayo®. Medicines in this class can slow stomach emptying, which may affect anaesthesia or deep sedation.
Active ingredient: orforglipron.
Each Foundayo® tablet contains 0.8mg, 2.5mg, 5.5mg, 9mg, 14.5mg or 17.2mg of orforglipron, depending on the strength you are prescribed.
For the full list of inactive ingredients, allergens and tablet appearance, read the patient information leaflet supplied with your medicine. If you have a known allergy or intolerance, tell us during your consultation so our clinicians can check the pack before we dispense.
Like all medicines, Foundayo® can cause side effects, although not everyone gets them. Digestive effects are most common when you start treatment or after a dose increase, and they often settle as your body adjusts.
Common Side Effects
- nausea
- diarrhoea
- constipation
- vomiting
- indigestion
- stomach pain or discomfort
Drink plenty of fluid, particularly if you have vomiting or diarrhoea, as dehydration can increase the risk of complications. Contact our clinical team if symptoms are persistent, severe, or make it difficult to keep fluids down.
Getting the Right Help
If you are unsure how serious a symptom is, use the guide below:
- Mild symptoms that are settling – contact our clinical team or speak to your pharmacist.
- Symptoms that are severe, persistent or worrying you – contact your GP or call NHS 111.
- Any of the red-flag symptoms below – stop taking Foundayo® and call 999 or go to your nearest A&E.
Stop taking Foundayo® and call 999 or go to A&E if you develop:
- severe, persistent stomach pain that may spread to your back, with or without vomiting – this may be pancreatitis
- swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, faintness, or a rapidly worsening rash – this may be a severe allergic reaction
- sudden loss of vision or a sudden significant change in vision
- severe constipation with stomach swelling, pain or vomiting, which may indicate a bowel obstruction
Contact your GP or call NHS 111 promptly if you develop symptoms of gallbladder problems, signs of dehydration, worsening kidney problems, or troublesome dizziness or low blood pressure. Do not wait for your next review.
Read the patient information leaflet supplied with your medicine for the full list of side effects. You can report a suspected side effect through the MHRA Yellow Card scheme.
Tell us about every prescription medicine, pharmacy medicine, vitamin, supplement and herbal product you take. Some medicines change how much Foundayo® your body absorbs, and Foundayo® can do the same to others.
Medicines that can increase your exposure to Foundayo®
- clarithromycin
- itraconazole or ketoconazole
- ritonavir or telaprevir
Medicines that can reduce your exposure to Foundayo®
- carbamazepine
- bosentan
- efavirenz
- rifampicin
- phenytoin
- St Johns Wort
Medicines Foundayo® can affect
Foundayo® may change your exposure to simvastatin, rosuvastatin and topotecan. Our clinicians may review your monitoring, the timing of your doses, or whether a different treatment suits you better.
If you use insulin or a sulfonylurea for diabetes, your risk of low blood sugar may be higher. Any change to those medicines must be made by the clinician responsible for your diabetes care.
Oral Contraception
Foundayo® may reduce the effectiveness of contraceptive pills. If you take an oral contraceptive, use a barrier method as well, or switch to a non-oral contraceptive, for 30 days after starting Foundayo® and for 30 days after every dose increase. Speak to a clinician or pharmacist about the option that suits you best.
Foundayo® is not right for everyone. Tell us before treatment if you:
- are pregnant, trying to become pregnant or breastfeeding
- have had pancreatitis
- have severe delayed stomach emptying or significant digestive disease
- have gallbladder disease
- have kidney problems or are prone to dehydration
- take medicines for diabetes, particularly insulin or a sulfonylurea
- have diabetic eye disease or unexplained changes in vision
- take treatment for high blood pressure, or often experience low blood pressure or dizziness
- have severe heart failure
- are due to have surgery, anaesthesia or deep sedation
- take any of the medicines or herbal products listed in the interactions section
Pregnancy and Breastfeeding
Do not take Foundayo® during pregnancy or while breastfeeding. Stop Foundayo® at least three weeks before a planned pregnancy and speak to your clinician about alternative care. If you become pregnant while taking it, stop and contact a healthcare professional.
Complete your consultation accurately, including any weight management medicine you have used before. The more our clinicians know, the safer the decision they can make for you.
If you are currently using another weight management medicine and are considering switching to Foundayo®, our clinicians can assess whether changing treatment is appropriate for you.
Foundayo® contains orforglipron. Wegovy® contains semaglutide and Mounjaro® contains tirzepatide. Although these medicines all help reduce appetite and increase feelings of fullness, they are different molecules and work differently within the body. For this reason, there is no direct dose equivalence when switching between them.
Many patients consider switching because they:
- Prefer taking a once-daily tablet rather than a weekly injection.
- Want to avoid needles or injection devices.
- Travel frequently and would prefer a treatment that does not require refrigeration.
- Would prefer a tablet with no food, drink or timing restrictions.
- Wish to explore an alternative treatment following side effects or personal preference.
Every patient is different, and our clinicians will review your current treatment, weight loss progress, medical history, treatment goals and any side effects before recommending whether Foundayo® is appropriate for you.
What Dose Will I Start On?
Because Foundayo® contains a different active ingredient to the other weight management treatments we supply, there is no fixed dose conversion between them.
When reviewing your assessment, our clinicians will consider:
- Your current treatment and dose.
- How long you have been receiving treatment.
- Your response to treatment and weight loss achieved.
- Any side effects you have experienced.
- Your medical history and any other medicines you take.
Depending on your individual circumstances, you may not need to restart treatment at the lowest Foundayo® dose. Your clinician will recommend the most appropriate starting strength and advise when to stop your current treatment and begin Foundayo®.
Do not take Foundayo® at the same time as another GLP-1 medicine unless a clinician has specifically told you to.
Things to Consider Before Switching
Clinical studies have shown that Mounjaro® has demonstrated average weight loss of up to 21% and Wegovy® Pill approximately 14–17%. Foundayo® has demonstrated average weight loss of approximately 12.4% in clinical studies.
However, the treatment associated with the greatest average weight loss is not always the most appropriate choice. Convenience, tolerability, side effects, lifestyle, personal preference and long-term treatment adherence are all important factors when selecting the right medication.
Foundayo® may be particularly suitable for patients who want a once-daily tablet with no food or drink timing restrictions, or who would prefer not to continue with weekly injections.
Our Clinicians’ View
For many patients, remaining on their current treatment may continue to be the most appropriate option, particularly if they are achieving good weight loss and tolerating treatment well.
However, if you would prefer a daily tablet, want to avoid injections, or feel Foundayo® would better suit your lifestyle, switching may be appropriate following a clinical assessment.
Our clinicians will carefully review your treatment history and discuss the potential benefits and considerations before recommending the option that best meets your individual needs.

We will review your current treatment, determine whether switching is clinically appropriate, and if suitable, recommend the most appropriate Foundayo® starting dose and switching plan based on your individual circumstances.
What is Foundayo® used for?
Foundayo® is a prescription weight management treatment for adults. It is taken alongside a reduced-calorie diet and increased physical activity to help you lose weight and keep it off.
Whether it is right for you depends on your BMI, any weight-related health conditions, your medical history and the other medicines you take. Our clinicians review all of this as part of your consultation.
Is Foundayo® an injection?
No. Foundayo® is a tablet taken once daily, so there are no needles, pens or refrigeration to think about.
It is still a prescription GLP-1 medicine, so it needs the same careful eligibility, interaction and safety assessment as any injectable weight loss treatment.
Do I need to take Foundayo® on an empty stomach?
No. This is one of the main practical differences between Foundayo® and other oral weight loss treatments.
You can take Foundayo® at any time of day, with or without food, and there is no need to wait before eating, drinking or taking your other medicines. Swallow the tablet whole and never take more than one dose in a day.
Which strength will I start on?
The usual starting dose is 0.8mg once daily for 30 days. Your clinician then decides whether to move you up to 2.5mg, and then through 5.5mg, 9mg, 14.5mg and 17.2mg, with a minimum of 30 days at each strength.
Starting low and increasing gradually gives your body time to adjust and helps reduce digestive side effects. Having used another weight loss treatment before does not automatically mean you will start on a higher strength.
Can I switch to Foundayo® from an injection or another tablet?
Often yes, following a clinical assessment. Foundayo® contains a different active ingredient to Wegovy® and Mounjaro®, so there is no direct dose-for-dose conversion between them.
Our clinicians will review your current treatment and dose, how long you have been on it, your weight loss so far and any side effects, then recommend the most appropriate Foundayo® starting strength and tell you when to stop your current treatment. Do not overlap two GLP-1 medicines or pick a switching date yourself.
What happens if I miss a tablet?
Take it as soon as you remember, as long as you have not already taken a dose that day. If the day has passed, skip it and carry on as normal the next day.
Never take two tablets in one day and never double your next dose.
Does Foundayo® affect the contraceptive pill?
It can. Foundayo® may reduce the effectiveness of oral contraceptives.
If you take a contraceptive pill, use a barrier method as well, or switch to a non-oral contraceptive, for 30 days after starting Foundayo® and for 30 days after every dose increase. Speak to a clinician or pharmacist about the option that suits you best.
Can I take Foundayo® during pregnancy?
No. Do not take Foundayo® during pregnancy or while breastfeeding.
If you are planning a pregnancy, stop Foundayo® at least three weeks beforehand and speak to your clinician about alternative care. If you become pregnant while taking it, stop and contact a healthcare professional.
What are the most common side effects?
The most common side effects are digestive: nausea, diarrhoea, constipation, vomiting, indigestion and stomach pain. They are most likely when you start treatment or after a dose increase, and they often settle as your body adjusts.
Drink plenty of fluid and contact our clinical team if symptoms are severe, persistent, or stop you keeping fluids down. Read the patient information leaflet supplied with your medicine for the full list.
How quickly will I lose weight?
In the ATTAIN-1 trial, adults taking the highest strength of orforglipron for 72 weeks lost an average of approximately 12.4% of their body weight. Around 60% lost at least 10%, around 40% lost at least 15%, and around 20% lost at least 20%.
Your own result will depend on the dose you reach, how well you tolerate treatment, and the changes you make to your diet and activity. Our clinicians will review your progress at each stage and discuss whether continuing on Foundayo® remains the right choice for you.
Can Foundayo® treat my type 2 diabetes?
Foundayo® holds a separate licence for type 2 diabetes, but the treatment we supply here is prescribed through our weight management service.
If you have type 2 diabetes, tell us during your consultation. Do not start, stop or change any diabetes medicine without the clinician responsible for your diabetes care, and be aware that insulin and sulfonylureas can increase the risk of low blood sugar alongside Foundayo®.
Do I need ongoing reviews?
Yes. Regular reviews let our clinicians check your side effects, your weight change and whether your current dose is still the right one before we supply more treatment.
Repeat supply is not automatic. If information is missing or your circumstances have changed, we will ask you for an update before we prescribe again.
